MPUSD has partnered with Colgate's "Bright Smiles. Bright Futures." to offer free dental check-ups for students ages 5 - 12.
In order for the dental bus to participate at your child's school, there must be 150 students registered at the school site with a consent form. If you would like your child to participate, please download this form, complete it and submit it to your child's teacher or health aide as soon as possible so our elementary schools can get an RSVP.
